Summary

This post is to support the Bee at home care team in optimising independence/home first. This is an assessing role which will include travel to service users’ homes and the hospital. A knowledge of SystemOne and the ledger system would be advantageous. The candidate will also be expected to cover care and support visits, reviews and work with the multi-disciplinary team to promote independence at home.

To work within the Domiciliary Care Service undertaking initial client assessments and screening initial referrals
received via Local Authority or directly from clients enquiring about services provided. The post holder will be
required to assess and determine the nature and level of care required by individuals referred to the service.
There will be a requirement to regularly review and update packages of care being provided.
